I acknowledge I will be participating in physical activity (strenuous or otherwise) during my participation in exercise classes,

personal training and/or educational workshops (collectively “IPG Sessions”) given by IPG Wellness Services Inc. (“IPG”)

held at the Saint Émilion, The Normand, Howard Grant, Les Terrasses Francesca and/or Relevé health & fitness centers in

Ottawa (the “Centers”), properties owned and operated by Lépine Luxury Apartments (“Lépine”) and/or Centurion

Property Associates (“Centurion”).

I have duly completed and executed the IPG approved Get Active Questionnaire governed by the Canadian Society for

Exercise Physiology (CSEP).

I acknowledge that there is an inherent risk of injury (both minor and serious) or death associated with participation in or

in proximity to athletic activities during IPG Sessions, including the use of the Centers whether for physical exercise or

otherwise.

I have considered and voluntarily assume all risk of injury or death relating to my participation in the IPG Sessions and the

use of the Centers and I hereby release and forever discharge and agree to indemnify IPG, Lépine and Centurion of and

from all claims, damages, actions or causes which arise therefrom.

I hereby waive all claims that I may have against IPG, Lépine and Centurion, their employees or agents for injuries I

may sustain as a result of any physical exercises or activities that I undertake at the Centers and whether or not

they are supervised.

I acknowledge and agree that IPG, Lépine and Centurion are not responsible or liable for any items that you leave in the

Centers or that are stolen from the Centers.

I agree that this waiver and release is reasonable and proper, particularly given the nature of IPG, Lépine and Centurion’s

business.

This Client Waiver Form shall be governed by and construed in accordance with the laws of the Province of Ontario and

the federal laws of Canada applicable therein.
